The responsible qualified professional should define which information is needed and how it is assessed. A generic internet form must not be treated as a complete screening protocol or permission to perform a procedure. Do not infer that a signed form removes professional responsibilities.

Define the purpose of each field

Separate contact and appointment information from health-related questions and clinical assessment. Collect only what is necessary for the defined purpose, through an appropriate process, and explain who can access it. Avoid copying a long checklist simply because another business uses it.

Fictional example: reception may need a contact number and appointment time, while a clinician needs access to the professionally reviewed assessment. Giving everyone the entire form for convenience can expose more information than their task requires.

Review the complete process

• Identify the professional responsible for the form and updates.
• Check that wording and consent match the actual service.
• Verify access permissions, retention and correction procedures.
• Test how a completed form is associated with the correct person.
• Keep sensitive details out of public pages and reminder messages.
• Do not publish before-and-after material without the appropriate authorization.

Digital does not automatically mean suitable

A digital form may improve legibility and retrieval, but only if the surrounding process is appropriate. Review security, access and contractual terms rather than assuming that an electronic signature or upload button establishes compliance.
